Effects of Diabetes

Similarly high blood pressure, increased blood lipid, renal disease, pregnancy, anaemia and smoking also have adverse effect in the progression of diabetic retinopathy.

Retina is the back portion of the eye. The optic nerve along with retina connects the eye to the brain. The high blood sugar damages the cells lining the walls of the arteries and the veins of the retina. These blood vessels may balloon in some corners or places to form micro-aneurysms that leak fluid, blood and fat into surrounding tissue. The fluid accumulation leads to thickening of retina. When it affects the retina known as macula, there will be a drop in vision.

Diabetes also damages the lens of an eye by opacities, it result in cataract. Diabetic patients develop earlier cataracts and may require surgery to replace it with a new intra ocular lens. If the blood sugar is brought under control and remains stable, vision will improve.


Timely treatment helps prevent further vision loss. Laser remains the mainstay of treatment for diabetic retinopathy. For fluid accumulation in macula, laser photocoagulation in focal or grid pattern can be applied. Laser treatment is often done to prevent complications related to diabetic retinopathy but not to improve vision.

Steroids and anti-vascular endothelial growth factor drugs are applied directly injected into the eye, these injections help reduce blood vessel leakage and formation of abnormal new vessels.

Hence Diabetic eye problems are treatable, if diagnosed early and also lifestyle management helps reduce the risk of developing type 2 diabetes. It can also slow down progression of pre-diabetes.

Symptoms of Diabetes:

  • In early stages there will not be any blurring vision.

  • If there is a accumulation of macula fluid, they will find a drop in vision.

  • Sudden deterioration in vision in advanced stage can occur due to bleeding inside the eye and retinal detachment.

  • Temporary change in refractive error.

  • Glare and blurring of vision could be noticed due to cataract.

How to Prevent Diabetes:

  • Strict control of blood sugar is the mainstay. Maintain HbA1C levels should be around six percent.

  • A healthy lifestyle including diet control and regular exercise helps avoid further complications.

  • Intensive control of blood pressure, renal function and serum lipid levels.

  • Blood haemoglobin should be within normal limits.

  • Avoid smoking.

It is typically made by a circular inflatable cuff connected to a gauge called sphygmomanometer or blood pressure cuff. The first aircraft of this type has been described by Dr. Scipione Riva-Rocci December 10 1896 in the Medical Gazette of Turin.

The current instrument is placed in the arm. A stethoscope is placed at the elbow to listen to the brachial artery. By inflating the cuff to a pressure higher than the maximum pressure, the artery of the arm is then occluded. It then deflates it very gradually and when the inflation pressure equals the systolic pressure (and maximum), the artery opens intermittently as manifested by the appearance of a knocking noise in the stethoscope; these sounds, called "sounds of Korotkoff, are due to turbulent blood flow, hampered by the cuff pressure. Pulses are also important perceptible by the patient and the examiner. When cuff pressure falls below the minimum pressure (diastolic), the artery is then permanently open: the laminar flow becomes turbulent and the auscultatory sounds disappear.

This is the method "counterpulsation.

You can also feel the pressure without a stethoscope, but less precise:

  • pulsation of the artery are reflected in the cuff, the cuff pressure oscillates between systolic and diastolic pressure, when the cuff deflates, it is therefore the pressure at which the gauge starts to oscillate (systolic pressure) and one to which the needle stops oscillating (diastolic pressure);
  • taking the pulse of the radial arm compressed when the cuff is compressed, it does not receive the pulse, when the cuff deflates, the pressure at which the pulse appears is the systolic pressure (this method does not d having diastolic pressure).

The association manual sphygmomanometer and stethoscope is increasingly being replaced by automatic equipment (electronic blood pressure) cuff inflated and engine noise is detected by a microphone (auscultatory method), or amplitude variations of 'wave pulse (oscillometric method). These devices allow repeated measurements and monitoring in near real time blood pressure.

One can also measure blood pressure by the same techniques but this time in the same wrist and finger. This is not however the reference method.

When a couple electronic sphygmomanometer to a battery and a memory, we can then take the blood pressure repeatedly during 24 h: the MAPA (= monitoring ambulatory blood pressure also falsely called Holter blood pressure).

In some cases, blood pressure is measured by inserting into the artery, a catheter filled with liquid and connected to a pressure sensor. This more invasive (and therefore riskier) gives more accurate measurements and gives the blood pressure dynamics. Thus, the pressure at each moment of each cardiac cycle the patient is measured, not only the systolic and diastolic blood pressure over time.

Protocol use

The standard method is to measure the cuff with stethoscope (counterpulsation). She is a patient in a reclining position, arms along the body, after five to ten minutes rest. The arm must be maintained at the heart: if the arm is too low, the pressure will be overestimated. The size of the cuff should be appropriate to the size of the arm if the cuff is too small, the pressure will be overestimated. Initially, we measure the pressure in both arms. Sometimes the pressure is slightly higher in one arm than another: we take the highest pressure as the reference value.

It is currently recommended during a consultation, measure blood pressure at the beginning and the end of the consultation: the syndrome of the white coat (the patient anxiety generated by the presence of medical staff) very often leads falsely high values early in the review.

Estimation without unit

In an emergency, lower systolic blood pressure can be estimated by simply taking the pulse, the latter resulting in the disappearance of a distal pulse; in an adult:

  • if the radial pulse is received, the systolic pressure exceeds 80 mmHg (T ? 8 mm Hg);
  • if the radial pulse is not seen but the carotid or femoral pulse is the systolic pressure is between 50 mmHg and 80 mmHg (8 cmHg ? T ? 5 mm Hg);
  • if the carotid and femoral pulse disappeared, the voltage is below 50 mmHg (T ? 5 mm Hg).

According to people, distal pulses (radial and pedal) may be more or less well received, and on the other hand, the disappearance of a pulse may also be due to external compression (effect withers) or internal (eg arteriosclerosis). We must therefore seek both radial pulse. On the other hand, if the voltage is below 80 mmHg, we did not feel the radial pulse, but conversely, if we do not feel the radial pulse, this does not necessarily mean that the voltage is low.

We can also estimate the systolic pressure with the "capillary refill" it exerts pressure on a nail, it fades, and watch how fast it recolors:

  • capillary refill normal (less than two seconds): systolic pressure exceeds 100 mmHg (T ? 10 mm Hg);
  • delayed capillary refill (greater than two seconds): systolic pressure is between 85 and 100 mmHg (10 cmHg ? T ? 8.5 mm Hg);
  • no capillary refill: the systolic pressure is less than 85 mmHg (T ? 8.5 mm Hg).


  • If it is always too high, it is a high blood pressure.
  • If it is too low, it speaks of hypotension.
  • If it is collapsed, it speaks of cardiovascular collapse, which can cause shock.
  • If it is increased in the presence of a physician, one speaks of the effect of "white coat"

  • Do not allow children's to play in rain.
  • Wear warm dress when entering to room.

We thought that Heart Attack occurs only for above 50years old persons. But nowadays even for 30years old person Heart Attack will be possible. 25% of people who are below 40years and 35% to 45% who are above 50years are suffering from Heart Attack problems. And even High BP is occurring in children’s who are between 14-15 age, this is due to children’s are not involved in regular exercise and setting in front of T.V and computer and eating with potato chips. So this will affect children’s health when they come to age 40.

The main reason for Heart Attack in Men’s is due to ‘smoking cigarettes ‘.Even for Style College students will be smoking cigarettes .And in women’s smoking and diabetes are the main reasons for heart attack. Peoples who are below 40years having habit of smoking, for them five times danger in heart attack.

People who are mentally affected, has sedentary life style and not doing regular exercise are the main reasons for heart attack.

Symptoms of Heart Attack:

There will be pain inside the chest and middle portion, shoulders, left hand, backbone and other parts, and even vomiting, sweating, respiratory problem, acidity and gastric are the other problems.

How to predict before Heart Attack occurs?

60% of patients will be suffering from chest pain, pain in left hand shoulder, stomach pain and sweating are the main symptoms for Heart Attack. And also 10-20 Heart Attack patients will say they are suffering from mostly back pain or non-digestion and discomfort in throat problems.  

And also there are chances of ‘Silent Heart Attack ‘.Silent Heart Attack means Heart Attack will occur without any chest pain. This type of Silent Heart Attack will occur whoever has more diabetes and also for old people.

How to Prevent Heart Attack?

Stop smoking cigarettes, balancing weight of your body, eat a food which has low fat and high vitamins. Controlling high BP and diabetes. Doing regular exercise. Men’s who are above 35years and women’s who are above 45 years should test once a every year.
